Position Description Performs supervisory and coordinative work in the operation of a specific program or departmental project. Responsibilities involve the coordination and supervision of the various functions and aspects of a specific program as well as the monitoring of program staff and operational procedures. Work is performed under general supervision with evaluation based on results obtained. May supervise program staff, volunteers and student assistants.

Major/Essential Functions

  • Make initial contact and provide information to the patient or the referring physician on behalf of a specific group of attending physicians

  • Responsible for the communication process with referring physicians to obtain referrals, authorizations, and medical records

  • Interview patient, develop evaluation of areas of concerns and anticipated needs upon admission and discharge including personal escort, transportation, housing, support care, equipment needs, transfer of records and appointments with physicians

  • Collect and assess information concerning the patient’s medical status from the referring physicians and the community. Assess social and financial situation of patients

  • Act as primary liaison among the hospital, the patient and family, referring physician, outside agencies, staff from other hospitals and the general public by providing Fax information and advice regarding hospital services

  • Coordinates inpatient and outpatient care by scheduling surgery, arranging diagnostic procedures, and/or coordinating requests for consultation among other medical specialties by following established protocols

Occasional Duties Ensure the outcome of treatment recommendations for continued care is conveyed to referring physicians

Required Qualifications

  • High school graduation plus five (5) years progressively responsible related experience

  • Additional related education may exchange for required experience on a year for year basis

Preferred Qualifications

  • Knowledge of basic office equipment and moderate computer skills

  • Knowledge of the various managed care plans and private insurances (MA, Tricare, Insurances, etc...)

  • Knowledge of referral process and out patient operations

  • Professional telephone skills, some familiarity with medical procedures

  • Communicate effectively orally and in written form, (Bilingual English/Spanish)

Minimum Hire Rate 12.42
